Do not submit this form until:
1. All users are trained
2. You have required hardware and your dealership is configured to meet the requirements specified in the Dealer Infrastrucure Manual.
Special Note - If you are replacing a new ID for an old ID, select YES to indicate this is a replacement ID request and enter the IDs to be removed in the Section : Cancellation ID.
Otherwise, select NO and enter the IDs in the Section : New ID.
